Damage From Hurricane Ida – Self Reporting Tool

In collaboration with affected counties, New York State has developed this online form allowing residents to self-report damages to their homes and/or businesses caused by the remnants of Tropical Storm Fred and Hurricane Ida. The purpose of this form is solely to collect information that may help State and local officials identify supplemental damages to develop and augment potential requests for available federal assistance programs.

This form is NOT an application for relief programs.

Please note:

  • Do not use this form to report immediate, life-threatening needs. Call 911.
  • Reporting damage is a voluntary activity and is not a substitute for reporting damage to your insurance agency, nor does it guarantee disaster relief assistance.
  • Please submit one survey per affected housing unit or business.
  • This form is for damages that occurred in New York State only.

Please be advised the Town of Callicoon has declared a State of Emergency beginning at 12:00PM, October 26, 2021 until further notice, due to the hazardous conditions during this period.

Of primary concern is the safety of our residents, highway employees working to keep our roadways cleared, utility crews restoring our electric service and our volunteer emergency service providers who will be responding to emergencies throughout our Town.

We will advise you if the conditions warrant any town road closurers.

A Tractor Trailer of Potable Water will be available until further notice across from the Youngsville School (1815 Shandelee Road), to any user affected of the Youngsville Water District. Please bring your own containers.

ACCEPTABLE: Solvents, oil-based paints, stale fuels, antifreeze, pool chemicals, acids, bases, poisons, fluorescent bulbs, fire extinguishers, smoke detectors, cleaning products, mercury-containing devices, pesticides/herbicides, automotive batteries, roofing tar, wood preservatives, adhesives, creosote, stains, inks. 2 forms of ID required.
UNACCEPTABLE: NO Latex paint, radioactive materials, explosives, ammunition, electronics, garbage, bulk liquids/drums/tanks larger than 5 gallons, LP/gas tanks, medical waste, tires, pharmaceuticals, refrigeration devices, NO commercial, industrial or agricultural waste
